Having identified the continued need for a GEH resource which requires minor or no modifications, you will now need to make those modifications, recording your changes in the Patient Information Review Form. You will need to update any existing leaflets to the current A4 template as dictated by the Trust Patient Information Resource Policy and Procedures.
Content should also be modified in mind of the standards for Patient Information Resources within the policy. These include the general principles that Patient Information:
- Should have a clear title to enable ease of location
- Should be evidence based
- Should be objective and not leading
- Should be necessary
- Should fulfill their purpose
- Should be composed from the patient’s point of view
- Should not discriminate
- Should be inclusive
- Should signpost users to other forms of information
- Should provide end users with details on how to provide feedback (this will be part of the standard templates)
- Should not include references (unless required for third party copyright). References will be retained in the associated form for supply if requested.

You will need to detail any content of this type used in the Patient Information Review Form and email patient.info@geh.nhs.uk with a copy of the resource and form for review.

Once you have reviewed your content, had it reviewed for copyright by the Knowledge and Library Service, you can now move on to Step 3: Approval.
