At this stage you should have an external resource, a new resource, or reviewed resource, and associated documentation (Patient Information Proposal Form or Patient Information Review Form) ready to send to your Directorate Governance Meeting for approval.

The Directorate of Governance Meeting will review your content looking at the below:
Please note by default all leaflets will be disseminated on the external website. If you do not want this, fill out the appropriate section of your form stating the reason why, for review by the Directorate Governance Meeting.
Once you have received final approval, send all the associated documentation to the Knowledge and Library Service at patient.info@geh.nhs.uk. The Knowledge and Library Service will set up your resource on the external website (or internal intranet) and will archive a version alongside the associated documentation.
You will now need to ensure all old and expired versions of the reviewed resource are deleted from folders and recycled if printed. It is a policy requirement that all patient information resources are only located on the external website (or internal intranet), which can be linked to, and only current patient information resources are used.
